Health Net Federal Services
We are committed to serving those who serve. We are in-network with Health Net Federal Services. Visits must be pre-approved by Health Net prior to beginning physical therapy. For more information, click here.
Workers’ Compensation
We accept workers’ compensation claims. We will contact your adjuster to authorize visits prior to beginning treatment.
Personal Injury Claims
We accept personal injury claims, such as from auto accidents. We will bill your auto insurance. If there is a lawsuit, we will have you and your lawyer sign a lien.
Medicare
We are in-network with Medicare. Physical therapy is included as part of health care benefits. Medicare covers 80% after your deductible is met. We will send claims to your secondary/supplemental insurance as well, if applicable.
For more information on Medicare, including limitations, click here.
Please note: if you are receiving any form of home healthcare (e.g. a nurse coming to your home to give you medications), Medicare will NOT pay for outpatient physical therapy until you are discharged by that home health agency.
Self-pay Option
If you do not have health insurance, there is a self-pay flat fee of $150 per visit.
